A Registered Nurse will visit with the patient and his/her family to make the initial assessment in order to determine the patient's needs for paraprofessional (Home Health Aide, Personal Care Worker) assistance. The Nurse will coordinate the care for the patient with the physician and family to provide continuity of patient care. The Nurse will then develop a plan of care for the paraprofessional to follow.
Services rendered by our paraprofessionals are supervised by the Nurse to assure the aide is performing his/her duties as indicated by the plan of care.
Neighbors Home Care employs Home Health Aides and Personal Care Workers. All of our Home Health Aides and Personal Care Workers must have a valid New York State approved certificate, meet all the health requirements per New York State regulations, and exhibit a caring and sympathetic attitude towards working with people.
The paraprofessional is able to assist the patient with activities of daily living, which can include bathing, shaving, toileting, dressing, mouth care, hair care, ordinary care of nails, walking and transfer activities, shopping, preparing meals and assisting the patient with eating; as well as light housekeeping duties in the patient's areas (such as changing bed linen, dusting and vacuuming, tidying the kitchen, washing dishes, tidying the bedroom and bathroom, and doing the laundry). They are also able to accompany the patient on errands and appointments. Although New York State regulations do not allow paraprofessionals to administer medications, the paraprofessional can remind the patient to take his/her self-administered medications as prescribed.
Based on the initial nursing assessment and the patient's needs, a plan of care will be established that will determine what tasks the paraprofessional will perform.
A Companion can act as a family substitute in an institutional setting to offer comfort to a patient while they are in the hospital or nursing home. Companions are not allowed to perform any personal care duties.
A paraprofessional can provide the needed services to a patient on a live in (24-hour) basis. This arrangement is available for patients who need assistance on an as needed basis during the night in addition to services during the day. The aide is available to ensure safety, provide patient supervision and toileting. The aide is provided with sleeping accommodations in the patient's home. However, the aide provides their own food.
